Understanding Informed Consent

At Mosaic Family Counseling Center Inc., we believe that healing begins with trust and mutual respect. One of the ways we build that trust is through a process called informed consent.

What is Informed Consent?

Informed consent means that you have the right to understand your care before you agree to it. Before we begin services, we’ll explain:

  • What therapy involves and how it may help

  • The types of approaches we may use

  • Possible risks or limitations

  • Your rights and responsibilities as a client

  • How your privacy is protected (and when it may be legally shared)

  • Fees, insurance, and cancellation policies

    We’ll answer any questions you have, in plain language, so you can make the choice that feels right for you.

Your Rights as a Client

  • At Mosaic Family Counseling Center, you have the right to:

    • Be treated with dignity, compassion, and respect at all times

    • Be seen and heard without discrimination, judgment, or bias

    • Receive services that are culturally responsive and trauma-informed

    • Understand your treatment plan and participate in decisions about your care

    • Know the qualifications of your clinician

    • Ask questions and receive clear answers

    • Decline or withdraw from services at any time

    • Request a different provider, or seek a second opinion

    • Have your privacy and records protected under state and federal laws (HIPAA)

    • Access your records, when appropriate, and understand how they are used

Your Responsibilities as a Client

To support your care and help create a safe, effective space, we ask that you:

  • Attend sessions on time or give advance notice if you need to cancel

  • Engage openly and honestly in the therapy process

  • Communicate changes in your contact, insurance, or billing information

  • Ask questions if anything is unclear or confusing

  • Respect your provider’s boundaries and the therapeutic space

  • Inform us if you are experiencing a crisis or urgent need for support

  • Be aware of our policies regarding safety, conduct, and technology use

You're in Charge of Your Care

You’re empowered: You can pause, stop, or change your care at any time.

You’re supported: We're here to collaborate with you—not to tell you what to do.

You’re informed: We'll keep you updated if anything changes about your services.
